Taken directly from the set of the NBC/Universal production, Battlestar Galactica, by Propworx staff.
Outfit worn by Number Six Cylon Natalie Faust prior to her demise. Natalie led the faction of rebel Cylons, but her fascination with Hera Agathon was her undoing. Worn by Tricia Helfer in the Fourth Season episode “Guess What’s Coming to Dinner?”

Worn in "Sine Qua Non"
Description does not address the two bullet holes in the center chest.
On that note, this dress may have been worn in "Sine Qua Non" as it features bullet holes where Sharon Agathon shot her, believing Faust was a threat to Hera Agathon. A second one was later cut open in an attempt to save her life in Galactica's sickbay.
May have been worn in earlier episodes and modified for later use, but this is not confirmable.
